As Ohio strives to become the country's most military-friendly state, military families deserve healthcare services that are second to none. CareSource proposes a one-year pilot program in collaboration with WPAFB to leverage CareSource personnel, technology and award-winning care model to implement community-based health management to military families with complex needs. CareSource is willing to commit the necessary funds for the execution of this pilot. $300,000 represents up to three FTE’s plus administrative, programmatic and technical support for this demonstration of enhanced community-based healthcare services supporting military families with special needs in the Dayton Region.

The CareSource model offers members access to a comprehensive continuum of coordinated, community-based, care services for families with special/complex healthcare needs. CareSource is proposing a one-year pilot program that will coordinate multi-disciplinary teams from across the region to implement community-based health management services by bringing an innovative government-industry partnership to families enrolled in programs like the Exceptional Family Member Program (EFMP). This mandatory enrollment program that works with key community agencies to identify active-duty sponsors who have family members with special medical needs will be the basis for a new approach to coordinated care. The CareSource approach, proven in government-sponsored programs across the nation, will significantly improve the continuity of care for military families across the Dayton Region with special needs.

The Home Base Pilot Program will remove the burdens and bureaucracies traditionally associated with these programs and streamline the system of care our military families receive when living in our military-friendly community. Using CareSource’s technology, award-winning care model, dedicated personnel and community know-how, the pilot program will demonstrate immediate healthcare improvements to existing programs while ensuring that no military family member living in the Dayton Region is ever overlooked or left behind. CareSource will lead the community effort while collaborating with WPAFB to kick-start the project. CareSource will fund the pilot. Through this government-industry partnership, “best practice” results from the pilot will enable both USAF and surrounding communities to communicate, collaborate, coordinate complex care and improve the health of our military families that need it the most, regardless of the geography of their next military assignment.

It is critically important to CareSource and the community that our military families are best cared for during their assignments at WPAFB. Ohio is making great strides to becoming the most military friendly State in the country - we must ensure that our community-based healthcare services, particularly in support of those with specialized needs, are the best of the best for those serving at WPAFB and bases/posts around the world.
