Resubmit: Home Base Pilot Program (with 2022 lessons learned) A pilot program for military members and their families serving in the USAF as well as other service member families with specialized/complex healthcare needs.
As Ohio is becoming the country's most military-friendly state, military families deserve worldclass healthcare services. CareSource proposes a multi-year pilot program in collaboration with OSD, DHA, and the military branches to include WPAFB that will leverage CareSource personnel, technology and award-winning care model to implement community-based health management to military families with complex needs.This initiative will improve health and well-being of our military families.

CareSource is willing to commit additional funds for the successful execution of this project. Funding represents need for Subject Matter Experts on the front end of the project for collaboration, discovery and formulation of pilot solution set. It also includes needed , programmatic and technical support that will result in enhanced community-based healthcare services that will support military families with special needs across the globe, including WPAFB and the Dayton Region.

The CareSource model offers members access to a comprehensive continuum of coordinated, community-based, care services for families with special/complex healthcare needs. CareSource is proposing a multi-year pilot program that will coordinate multi-disciplinary teams from across the DoD to implement community-based health management services by bringing an innovative government-industry partnership to families enrolled in programs like the Exceptional Family Member Program (EFMP). This particular enrollment program works with key community agencies to identify active-duty sponsors who have family members with special medical needs will be the basis for a new approach to coordinated care. The CareSource approach, proven in government-sponsored programs across the nation, will significantly improve the continuity of care for military families across the DoD with special needs.The program will include a collaborative front end (phase 1) that will enable fruitful discovery of issues associated with coordination care for the EFMP program. This partnering will enablethe removal of burdens and disconnects traditionally associated with cross agency programs and ultimately identifying, solving and streamlining and demonstrating an improved system of care our military families receive regardless of where they are stationed around the globe. Using CareSource’s technology, award-winning care model, dedicated personnel and community know-how, best practices industry models may be used to solve and deliver healthcare improvements to existing programs while ensuring that no military family member arriving in the Dayton Region or across the country is ever overlooked or left behind. CareSource will lead the effort while collaborating with OSD and XXX to kick-start the project that will include two phases: 1) a collaborative consulting engagement for problem identification and recommendations, and 2) demonstration of pilot project based upon phase 1 findings. Through this government-industry partnership, “best practice” results from the pilot will enable both DoD and each duty station/base surrounding communities to communicate, collaborate, coordinate complex care and improve the health of our military families that need it the most, regardless of the geography of their next military assignment.

CareSource is willing to commit additional funds that may be needed to ensure the successful execution of this project, as it critically important to our military families. $10M represents all necessary healthcare Subject Matter Experts involved in phase one "front-end" of the project and also includes administrative, programmatic and technical support for the phase two demonstration of enhanced community-based healthcare services supporting military families with special needs arriving at WPAFB or any other military installation around the globe.

CareSource’s headquarters are in Dayton, Ohio, home of Wright-Patterson Air Force Base. Many of our employees are military families and we have a passion for not only investing in the Dayton Region but in the infrastructure that supports our employee base. We also serve veterans through our government-sponsored health care programs we manage across the nation. CareSource has applied lessons learned in the past year to this application, and the need to answer the call for solutions from Secretary of Defense Austin who identified the need for DoD improvements in these programs across DoD military departments and respective agencies. CareSource offers best-of-breed industry programs that will bring proven positive health outcomes to help ensure that our military members’ families, living in our local communities, are not overlooked when it comes to continuity and quality of medical care and services, particularly those with special needs. It is critically important to CareSource and the community that our military families are best cared for during their assignments at WPAFB. Ohio is making great strides to becoming the most military friendly State in the country - we must ensure that our community-based healthcare services, particularly in support of those with specialized needs, are the best of the best for those serving at WPAFB and bases/posts around the world.
